Canadian Methadone Survey

The Canadian Methadone Survey was created by the Canadian Association of People Who Use Drugs (CAPUD) and the Urban Survivors Unions (USU) to gather data on take-home methadone doses in order to assess benefits and impacts on patients. Data collected in the United States and in Canada will be analyzed and compared.
This particular survey is only for people on Methadone in Canada, OAT prescribers are kindly asked to share with their patients.
